Wiki ANOSCOPY & E&M svc done same day

discussion has been circulating that an ansocopy is part of the exam of the patient & should not be billed separately of the E&M.
I have a Colo-rectal Surgeon that completely disagrees & I am try to find CMS or other credible documention to support it one way or another.

It has always been my understanding that if the pt comes in for an office visit and they decide to do the procedure then a 25 modifier can be used on the ov and both billed. If the procedure was scheduled, then just the proc.
