Wiki Anthem paying 83861

Is anyone else having issues getting the 83861 paid through Anthem BCBS? I have tried every modifier to get the 83861 when it is done bilaterally. I billed as QW, RT, 59 and QW LT and they paid a few then denied as per Medicare you can not bill the RT, LT modifiers with this code. I have then tried 83861 QW, 50 and one paid and one denied as CO4. I don't have any issues with any other payer. Any help would be greatly appreciated.

I would recommend reaching out to Trukera to get the reimbursement guide for the 83861 as it shows what the majority of major carriers are wanting for the tear osmolarity test.
