for 6 weeks post total joint replacement, we keep patients on Coumadin. Can we bill CPT 99363 for INR management? apparently, Family Physician's are unable to bill for these because it is bundled with their attached E/M. Does this mean Medicare does not allow it at all, or just with an E/M?

Per CPT guidelines for this code, it must be 90 days with a minimum of 8 INR measurements. Therefore, 6 weeks would not meet the criteria for this code.
