Hi!

I am looking for information on the anticoagulation therapy codes and how they work....or in what situation they can be used. I have not found anything that makes me confident that I understand them correctly so I am hoping someone can help me.

If the nurse dose the dosage changes and interprets the INR testing. They do this by using a standardized procedure that states if the INR is (?) the dosage is (?).

If the INR is out of range, they must communicate with the physician before adjusting the dose.

If the at least 60 continous days; first 90 days and other criteria is met can the 99363 be coded for this situation?
