Go to Yahoo, then search for your state's Department of Insurance, Division of Worker's Compensation. When you find the site, search within the webpage for 'physician reimbursement for special reports'. It'll tell you what to do - Work Comp varies from state to state. We use 99080, but I'm not sure how much the reimbursement is; it's really not my area of expertise. :p
