Wiki Anyone have a Dr. who writes the perfect notes?

Does anyone here have a dream Dr. who writes the perfect notes? Clearly lays out the workup they did, clearly outlines their separately identifiable E/M, specifies when conditions are chronic or acute, specifies when cancer is being treated or prevented, always lists how much time they spent etc., etc?
