How would you code this dx?

Severe calcific aortic valve stenosis in the setting of a previously repaired bicuspid aortic valve in 2008.
Moderate-to-severe aortic valve insufficiency.

Would this be Q23.1 or would this be coded as I35.2, Z98.890 or a T code?
