Some insurances seems to pay for "application of cast" and material when is done at the same time as the procedure and other insurance companies consider it "bundled" in the procedure. Is there a rule?

Per CPT, the application of an initial cast at the time of a 'restorative treatment or procedure(s)' is not separately billable. See the instructions under "Application of Casts and Strapping" on page 139 in the 2010 CPT Professional Edition for guidance on this.
