Hello
I have heard that AZ Medicaid will not pay for revascularization codes (37220-37235) in a POS 11 but I cannot find anything on their website. Can anyone help me?

I worked at Arizona Heart until very recently. These procedures are not done in the office. They are only performed in POS 22 or 21.
