I have a doctor that bills for ASC and his professional billing for same DOS and same codes. Exp: 64493, 64492 with 50 modifier on both. Both POS are 24. One under Ambulatory Center the other under the doctor prof . Can this be billed like this and paid? I have every insurance pay one and not the other. But I am being told both should pay.
 
I get either paid already under another NPI / Tax ID or it denies under Dr NPI as provider not credentialed to provide service. Which he is on these insurances.
