Wiki ASC Commercial payors reimbursement

I am wondering if ASCs receive separate reimbursement or payments for implants in cases such as cervical spine fusions and total knees etc. Or does the contract include everything in the bundle. When I use Find-A-Code to look up ASC payment calculators, it seems like reimbursement is rather low and they don't really provide commercial payer contracts just Medicare.
