Wiki ASCAN coding question: unilateral vs bilateral cataract

One of our providers has informed me that ASCAN coding should always reflect a bilateral procedure. The problem for me is that I often see this procedure performed bilaterally even when the cataract diagnosis is unilateral (H25.12, for example). Does anyone know if it's okay to link the 76519 procedure code to an eye w/ an intraocular lens (Z96.1) instead of a cataract? I can't seem to find this answer anywhere.

With Medicare patients, we always bill out 76519 2x (once with mod. 26 for the professional component). All other payers have required RT and/or LT, however. So I'm not sure how that would work for a bilateral procedure if the cataract only impacts one eye...
