The doctor performed aspiration of a musculotendinous cyst of the rotator cuff by "taking a spinal needle with a control syringe and aspirating any area of the cyst noted on the MRI in that location of the rotator cuff.

I have considered 20610, but it's not technically the joint. It's also not a bursal cyst or a ganglion. Does anyone have any suggestions as to what code to use in this situation?

Good question. I would probably use 20612. Musculotendinous cyst meets the definition for this in my opinion. Ganglions arise from tendon sheaths.
