hi,
correct me if i am wrong . If a physician assess a problem whether it's a new problem or an existing problem he can bill for it correct.
 
Are you talking about putting it on a claim as a diagnosis? Or do you mean "assess" as in do an evaluation and management service???? Please explain further

I am under the impression that if it is addressed that it can be billed as a diagnosis on the claim form.

Assessment Plan

If the ONLY place the problem is "addressed" is in the Assessment/Plan then I would not code it.

If the E/M of the problem was documented in the body of the note (history, exam and MDM), then I would code it.
