Wiki Assistance please Ortho coders....patellectomy?

Hello all you Ortho gurus! Looking for an appropriate CPT code for a patellectomy due to hardware failure after a TKA. Pt has already had if reconstructed (27422), now it is time to remove it. DX T84.022-Instability of internal right knee prosthesis.
What would you use? Excision?

Did they leave the native patella in when they did the primary TKA or resurface it, is there a button? It depends on what's in there now native vs. prosthesis/hardware/implant.

Thank you for the reply! It would appear in 2019 in was resurfaced, 2021-the prosthesis was implanted . I can not find anything in the documentation referencing a button.

Interesting. It's difficult without the op note. So they have a TKA (including a patellar component) in place and they are swapping out the patellar component only or just taking it out/no swap? Depending on the details it could be 27486 (if explant/swap with new). If not, maybe 27350 if they still have some native patella. You might even look at 27438 but that's only if native. Did you check CPT Asst or Coding Clinic?

At this point just trying to get a code for authorization. The intention is no swap.
