Assisted living facility - PLEASE HELP

CAN SOMEONE PLEASE HELP??

How would a physician bill for professional services provided to a patient at an assisted living facility which does not have an NPI? The physician has his own office and NPI. Would this follow the Medicare Global part A rules? We are stumped. Everytime we think we might have an answer, we come back to the lack of the facility NPI. Help please?
