Is there a CPT code for asthma control test form or is this included in E/M treatment plan for medical decsion making?

We include the completion and scoring of the ACT in the E/M for asthma visits. There is not a specific CPT to represent this service. It is more of a questionaire given to the patient/parent to complete rather than a test.
