I have a provider who has stated aortoiliac occlusive disease(i74.09) in his op note for bypass procedure. Does the provider need to state that the patient has an embolism or thrombosis to use this code.
The debate is that we should code atherosclerosis(i70.9) since that is the disease process warranting the procedure. .
Provider does state that severe atherosclerosis of aorto iliac caused embolic foot. But then coded in H&P and Opnote aortoiliac occlusive disease.
Any thought would be helpful
