I have been working in an Outpatient Physician Group for less than a year and noticed a trend that I have not seen before in other jobs. I constantly see I48.0 Paroxysmal Afib as a dx and I am concerned that it is being used improperly, without enough documentation, and am wondering if it is being used as a default code. My understanding is that it occurs for less than 7 days, and can occur repeatedly for quite a long time. Does the length of time that it has been going on affect the coding? Or is is always coded I48.0 as long as episodes are under 7 days? I am not seeing reports documenting the timing. Please advise. Thank you!
