attempted implant of BiV ICD

Hello coders-
I have attached the op note in hopes that someone may be able to help me code this one. My doc attempted to implant a BiV ICD for a patient who already has a pacemaker. It was unsuccessful & he ended up implanting a dual chamber ICD. What can be coded?
