pt.came in w.nstemi and dr.did lhc w.angiogram and decided to perform pci to lad. placed 2 guidewires and after crossing bifurcation lesion and upon further eval.of angio it was revealed treatment would require rotational atherectomy and d/t pt's nstemi it was felt rotational atherectomy would be high risk so attempt was stopped. Would it be correct to bill a 92941 with a -53 modifier, even though no actual intervention was started (as they only inserted the 2 guidewires)--or should I just bill the 93458 and maybe try for a -22 in hopes of getting increase payment? Any thoughts?
