Good afternoon,
Question posed "can attestation information be used to code an encounter note?". Providers are asking for written guidance on this and I am finding conflicting information. Does anyone have definitive guidance I can use for provider education? I also need to be able to cite the source. This is for outpatient office visits with an FQHC facility. Very much appreciate any clarity on this issue. Thank you

Hello, I am not an expert but we do code notes with MD's attestations. This question was never raised maybe because logically MD is the one who documents the most correct, final dx and he is the one who confirms resident's information and then puts a stamp/attestation. So, we do code from MD's attestation as the most accurate dx. Unfortunately, I don't have any sources to confirm it.
