Does anyone have an audit form that they would be willing to share?

I am looking for a professional tool that I could use to report the results to the physicians as well as a cover letter

Send me you email address and I'll send you an "audit results template" ... it's an Excel spreadsheet that we use to report results to our doctors.

It may not be perfect for you, but it'll give you a start.
