Audit guidelines for hydration

Patient comes in for labs and receives hydration or they are not feeling well and come in for hydration. For audit puposes does a physician, NP, PA have to draw up an order? The nurses are documeting that patient is here for hydration purposes, is that enough? Thanks in advance.
