Hello, when establishing audit methodologies, I would like to understand if others are capping certain findings at 1, when the error was identified more than once. For example, if 3 services are over-coded by one visit level, we count this as one error from a scoring perspective. (the same error made multiple times). Appreciate the feedback.
 
I would count these as 3 errors. I audit internally and our goal is to be helpful internally before it is discovered externally. Any outside payer would likely count these as separate errors also.
