Need the brain trust to weigh in on these please:

For HPI would you count "pain does not radiate down the arm" as associated signs and symptoms? I usually like to see positive findings for this category not the absence of something? Should it be ROS under musculoskeletal?

How about "no other complaints or symptoms" - can that be interpreted as "all other systems negative" For ROS.? My co-worker really bugged by that one.

Would anyone allow this to count?
"11 point review of systems was performed and is negative unless stated in HPI?
My issue with that is ROS and HPI should not overlap,if they are there for a broken finger then ROS can't include musculoskeletal since the is the HPI location element?

Is it just me or does anyone else think there it a problem that you can have a comprehensive exam by reviewing 8 organ systems- could be just one element each, but if you do 7 with just one element each it doesn't meet even a detailed exam? 1995 guidelines don't specify what you have to say just that you did for comprehensive.

Any thought appreciated!
