When auditing the level of office visit...if the PFSH is only reviewed, but not updated, can you still count it towards the level off office visit? Thanks!

The notation must read indicate from what date the previous history was reviewed and that no changes were required such as...

Past medical history reviewed from 01/22/2014 and not changes required.
 
I must agree, if PFSH only states reviewed, I would not count it as reviewed, it must have that day's date stating that it was reviewed and when it was reviewed and of course if any changes.
