Hi,
I have been going through the Medical Record Auditor exercises and I have a problem with the E/M. The format is done in paragraph form and the HPI and ROS are not easily determined. How can you tell the difference when it does not state, "patient denies," etc.. My answers are different from the exercises and I cannot see where there is an obvious line, more like opinion. Thanks so much for your help in advance!
