I see that the physician must perform one of the 3 key components in it's entirety (exam, history, or MDM) or spend more than 50% of time on the visit. Our physicians are going with one of the 3 components. My question is, the physician addresses the component in his attestation but it's also documented within the note completed by the NP. I see MDM documented in the note by the NP and different MDM documented by the physician in his attestation. Shouldn't all of the component be documented in his attestation in order for him to be able to bill under his NPI? Thank you!
