I am looking for clarification regarding billing/coding for ANS testing, CPT codes 95921/95923. Some of the MACS have LCDs but Noridian (Arizona "F") does not. The codes are listed on the Noridian fee schedule. I am wanting to know how to follow billing guidelines without an LCD or NCD. Any advice would be greatly appreciated.
