Can anyone help me clarify documentation requirements for the AWV? I've tried searching various resources but have been unable to find any guidance on my questions. Specifically: If a patient is physically/mentally unable to complete the HRA on their own, can the provider go through the questions with the patient and immediate care givers? Also, when listing current medications, is it acceptable to list "See MAR" instead of listing out each med? If it's noted that the patient has dementia, is that acceptable as the cognitive function assessment?
