I am looking for the website which would specifically outline what is "mandatory" for the completion of the Medicare AWV. I would assume that there are detailed regulations governing this service but am unable to find it on Medicare's website.

In specific I am looking for guidance as to the following subjects:
1. Do you have to complete a ROS and/or PE for an AWV patient
2. Do you have to scan the completed ppw, which will be in the patient's handwriting, stating their opinion of their condition

Thank you for any assistance and/or guidance which anyone can give me on this topic.

Thank you very much for the link. I have previewed that link but do not find anywhere, in writing, whether a ROS and/or PE is required in order to meet billing requirements for AWV. Could you please elaborate on that.
 
Normally a ROS and Exam would warrant an E/M code. If the provider preforms a AWV, and the patient has problems that are addressed during the AWV exam, the physician can bill an E/M code, plus an AWV.
