Hello,

A patient was diagnosed with HIV in 2019 and the provider was using B20 as the primary Dx. The patient has been on antiretrovirals since 2019. The provider is now using the Z21 diagnosis as primary since 2024.

The provider specializes in HIV and only sees patients for HIV related illness and management of medications. Would this be correct to keep using the Z21 or should the provider be using B20 if he is managing specifically HIV medications.

From ICD-10 Guidelines
"Patients with any known prior diagnosis of an HIV-related illness should be coded to B20. Once a patient has developed an HIV-related illness, the patient should always be assigned code B20 on every subsequent admission/encounter. Patients previously diagnosed with any HIV illness (B20) should never be assigned to R75 or Z21, Asymptomatic human immunodeficiency virus [HIV] infection status."
