Wiki Baby and Placenta delivered without Doctor present

If the note states that the baby was delivered by the RN and the placenta was delivered spontaneously and the physician was not present during either, can the hospital still bill a facility charge for the delivery ? If so, what would we bill and couldyou guide me to the documentation to support so I can provide to my HIM director?
 
If it is a commercial insurance you can still bill global, however if it is Medicaid, you can only bill for visits and repairs done by the MD.
 
Just to clarify, I am looking at the ICD10 procedure code for the hospital to bill, not the professional charges for the phsycian.
