Wiki Baclofen Pump Refill

Would it be more appropriate to bill J0475 (Injection, baclofen, 10mg) or J3490 for a baclofen pump refill? I'm leaning more towards J3490.

The drugs used are: Baclofen. Sodium Chloride & Sterile Water.

Any input would be greatly appreciated.

The HCPCS code for Baclofen 10mg is J0475. J3490 is unclassified drug, insurance wouldn't recognize which drug is being injected.
