Both of our bariatric surgeons insist that the dx codes can be pulled from all previous dates of service including those of other providers. They do not document the qualifying dx codes in their op report and sometimes their consult report. They are only putting E66.01.

Each services needs to stand on it's own with op report or progress notes just like any other services, right?
