Patient comes into GI clinic for a follow up of Barrett's esophagus. Biopsies done and the pathology report comes back stating no metaplasia, dysplasia or cancer. Would you code Barrett's as an existing condition or use the History of Digestive Disorder code. I am with the understanding that once you have Barrett's, you always have Barrett's. We use Provation and the system just lists all of the diagnoses at the end of the report.
