My injection fee for giving (96372) and medicine(J1100) are getting denied with rejection code 45. Per remit pt is not responsible for it. Oh, and I'm billing 99213-25
also, which is being paid. I didn't have this rejection last year. Any suggestions would be greatly appreciated!
