Wiki BCBS Denial for 46200 with 46505 - Fissurectomy with Botox Injection?

Has anyone had experience with getting 46505 (anal sphincter chemodenervation) paid when it's billed with 46200 (fissurectomy)? My resources say they should be billable together without a modifier, but BCBS is denying 46505 as bundled. Any help would be greatly appreciated, thank you!
