the modifier -25 was forgotten on ov , Bcbs now is denying original billing and corrected claim with -25 on ov with remit code N58, N23 stating pt liability issues. We have had this problem 2x now with South Carolina Bcbs being the common thread.
Anyone have any suggestions ????
