I am relatively new to coding behavioral therapy notes for outpatient therapy. I am unsure how to update the diagnoses for billing. Do I code according to the therapy notes or do I carry over previous diagnoses? For example: The patient has diagnosis of Bipolar disorder, current episode manic without psychotic features, severe F13.13, f41.9, G47.00. The next therapy note states they have psychotic features. However, they just copy and paste the previous diagnoses. Do I then update the diagnosis to F13.2. I know this may sound silly but I am unsure if I should be coding per each note for billing or for the original diagnosis from admission. Also, do I just code for each session per notes since they are seen every month.
