I have a question of how category V15.4 is used. Is the excluded code range of 290-316 for a current condition or a history of (past) condition? I have a patient for which that code range is a current condition but I need to use a secondary diagnosis to give the insurance company a reason why additional sessions are necessary and the V15.4 category seemed perfect except for the exclusion.
