In regards to the operative field if a patient comes in with a stab wound to the upper arm and the doctor extends the wound both proximately and distally. Through this wound he repairs an artery. Wound closure was then performed. The preexisting wound measured 2 cm and it was closed in a layered fashion. The proximal and distal extension was similarly repaired.
Can a closure be billed since the wound became the operative field

Can closure of the preexisting 2 cm wound be coded separately.
