Does anyone know if the supervisor needs to sign off on every bh note while the therapist is doing clinical supervision?
 
From my experience, it only needs to be signed by the clinician, not the supervisor. However, I feel it's a good idea to have them sign off on the items that must go with medical records. Main ones being biopsychosocial and treatment plans.
