Looking for advice on behavioral health claims. Many of our claims require an insurance referral. Presently, our BH has created a template which she will complete that answers all questions in the referral form. This is tedious and requires our staff to notify her when a new referral is needed. My question is, shouldn't the information in the visit note contain all the information necessary to complete a referral?

Since you've identified the information necessary, then yes, the provider should be documenting what is needed in the visit note.
