I have a provider that has extracted the complete Bi-V ICD system, including all three leads due to infection. I am billing 33241 for the generator removal and 33244 for the dual lead extraction. Does anyone know if there is an appropriate code for removal of the LV lead, or would it be included in the 33244?

I think it's bundled into 33244 since the CPT books states Leads 9s0.
