For bilateral breast biopsy done with the same guidance, we are coding the primary code 2x with LT and RT modifiers. We are getting denials for the second code and are being told that we should be billing the first breast with the primary code, and the other breast with the add on code for additional lesion. What is the correct way to bill a bilateral breast biopsy?
 
Lesion on the other breast is coded as "additional lesion" if performed with the same guidance.
