Wiki Bilateral venogram + 75827 ???

Would you bill 36005, 36005-59, 75822 and 75827 (because central veins were evaluated as well?)

Procedure: Bilateral upper extremity venogram.

Indication: Preparation for dialysis access.

Results: Patient was identified and brought to the vascular unit. The right and left arms were prepped and draped in the usual sterile fashion. Percutaneous intravenous catheters were placed in the antecubital fossa bilaterally. Using digital subtraction angiography and hand injection, bilateral upper extremity venography was performed. Central veins were evaluated as well. At the completion of the case both catheters were removed and pressure was held for 10 minutes without incident. Patient tolerated the procedure well and left in stable condition.

Findings are as follows: The left upper extremity shows a patent brachial vein. The basilic vein and cephalic vein are occluded. The axillary vein is patent. The innominate vein is patent. The superior vena cava is patent. The right upper extremity shows a patent brachial vein. The cephalic vein and basilic vein are occluded. The right axillary and subclavian veins are patent. The superior vena cava fills from the right as well.


Result Impression


Occluded bilateral upper extremity cephalic and basilic veins. Patent brachial veins, axillary veins, subclavian veins, and central veins as described above.
