Wiki Bill as one Excision or two????

A patient has two separate lesions that need to be excised and they are in very close proximity on the back (both are moderately dysplastic nevi) ? the provider will excise both with the same cut. So, would it be billed as two separate excisions or as one large excision?

It is clearly two separate lesions and the borders are not touching, but are still very close.

Can anyone help?

If it is performed as one excision encompassing two lesions the it is billed as one excision code using the largest excised diameter.

My suggestion would be code as a single larger lesion excision
