If the patient was told to come back to get his flu shot in two days because he has a fever on the original visit, do we code an E/M for him when he comes back for the flu shot?. Or do we just code for the injection and flu shot? No E/M? Or would it be an established patient E/M. ?

No E&M as the only reason to return is for the injection.

if encounter follow-up is only for flu and no E&M done then code only flu.

No, and E/M would only be appropriate if it was a new problem in this case

yes, it is a new problem its right to assign a E&M level when the patient came with fever on the original visit, But incase of follow-up for vaccine encounter for a established problem like this case, we need to append only the vaccine.
